Abstract
Με το πέρασμα του χρόνου, οι ανθρώπινες δραστηριότητες και οι αλλαγές στον τρόπο ζωής, έχουν επιφέρει αυξημένη παραγωγή διαφορετικών τύπων αποβλήτων. Από την εξέλιξη αυτή δεν θα μπορούσε να αποτελεί εξαίρεση ούτε ο τομέας της υγείας.
Ιατρικά απόβλητα (ΙΑ) χαρακτηρίζονται όλα τα απόβλητα που παράγονται εντός των χώρων υγειονομικής περίθαλψης και διαχωρίζονται σε δύο μεγάλες κατηγορίες, τα Επικίνδυνα Ιατρικά Απόβλητα (ΕΙΑ) και τα μη Επικίνδυνα Ιατρικά Απόβλητα (μη ΕΙΑ). Ποσοστό 75%-90% της συνολικής παραγωγής ΙΑ παγκοσμίως, υπολογίζεται ότι είναι μη ΕΙΑ, ενώ το υπόλοιπο 10%-25% υπολογίζεται ότι είναι ΕΙΑ και πιο συγκεκριμένα, μολυσματικά, τοξικά και ραδιενεργά.
Σε πολλά νοσηλευτήρια, ιδίως σε αναπτυσσόμενες και μη ανεπτυγμένες χώρες, η διαχείριση των Επικίνδυνων Ιατρικών Αποβλήτων Μολυσματικού Χαρακτήρα (ΕΙΑ-ΜΧ) αλλά και όλων των τύπων ΕΙΑ δεν είναι η ενδεδειγμένη ή ακόμα χειρότερα είναι ανεξέλεγκτη, κυρίως λόγω περιορισμένων οικονομικών πόρων. Το ζήτημα της διαχείρισης των ΕΙΑ-ΜΧ αποτελεί σημαντικό κεφάλαιο όσον αφορά την προστασία της δημόσιας υγείας και παράλληλα αποτελεί περιβαλλοντική και οικονομική πρόκληση. Περιβαλλοντική γιατί τα ΕΙΑ-ΜΧ περιέχουν επικίνδυνους παθογόνους μικροοργανισμούς, με πολύπλοκη σύσταση οι οποίοι αν δεν τύχουν της κατάλληλης διαχείρισης και επεξεργασίας, προκαλούν αρνητικές επιπτώσεις στο περιβάλλον. Οικονομική γιατί η ορθή διαχείριση των ΕΙΑ-ΜΧ έχει πολύ υψηλό κόστος σε σύγκριση με τη διαχείριση των μη ΕΙΑ, λόγω της ειδικής επεξεργασίας που χρειάζεται να υποστούν για να μετατραπούν σε μη ΕΙΑ και να καταστούν ασφαλή για απόρριψη σε χώρο υγειονομικής ταφής.
Κύριος σκοπός της παρούσας διδακτορικής διατριβής, ήταν η επικέντρωση στη βελτιστοποίηση της διαδικασίας διαχείρισης των ΕΙΑ-ΜΧ σε 6 τμήματα του Γενικού Νοσοκομείου Λευκωσίας (Γ.Ν.Λ.). Επίσης, επικεντρώθηκε στη βελτίωση του επιπέδου γνώσης, στάσης και συμπεριφοράς των κύριων επαγγελματικών ομάδων που εμπλέκονταν στη διαδικασία διαχείρισης των ΕΙΑ-ΜΧ.
Για την εκπλήρωση του σκοπού και των στόχων, χρησιμοποιήθηκε η μεθοδολογική τριγωνοποίηση με τη χρήση ποιοτικής και ποσοτικής μεθόδου, ενώ αξιοποιήθηκε η θεωρία του μοντέλου PDSA (Plan-Do-Study-Act), γνωστό ως κύκλος του Deming. Η συλλογή των δεδομένων πραγματοποιήθηκε μέσω επιτόπιων παρατηρήσεων, ερωτηματολογίου και ανασκόπησης εγγράφων. Επίσης, πραγματοποιήθηκε καταμέτρηση της παραγόμενης ποσότητας ΕΙΑ-ΜΧ σε δύο διαφορετικές περιόδους από τα υπό μελέτη τμήματα.
Βάσει των αποτελεσμάτων του ερωτηματολογίου με δείγμα 82 συμμετέχοντες καθώς και των επιτόπιων παρατηρήσεων, συμπεραίνεται ότι η γνώση του προσωπικού όσον αφορά το θέμα των ΕΙΑ-ΜΧ αλλά και των πρακτικών που εφαρμόζονταν στα υπό μελέτη τμήματα ήταν μέτρια και έχρηζε σημαντικής βελτίωσης. Επίσης, διαφάνηκε ότι οι παράμετροι που προστέθηκαν στη διαδικασία και εφαρμόστηκαν στα πλαίσια της παρούσας διδακτορικής διατριβής, είχαν θετικό αντίκτυπο στη γνώση, συμπεριφορά και στάση του προσωπικού σε σχέση με τη διαχείριση των ΕΙΑ-ΜΧ που εφαρμοζόταν. Θετικό αντίκτυπο, είχαν και στη συνολική παραγόμενη ποσότητα ΕΙΑ-ΜΧ από τα υπό μελέτη τμήματα, όπου παρουσιάστηκε σημαντική μείωση περίπου 11% στην παραγωγή ΕΙΑ-ΜΧ ανά κιλό/κλίνη/ημέρα.
Η μεθοδολογική προσέγγιση που εφαρμόστηκε, μπορεί να χρησιμοποιηθεί σε τμήματα με παρόμοια χαρακτηριστικά με τα υπό μελέτη και από άλλα νοσηλευτήρια στην Κύπρο ή το εξωτερικό για αξιολόγηση και πιθανή βελτίωση των διαδικασιών και πρακτικών που εφαρμόζουν. Οποιοδήποτε νοσηλευτήριο μπορεί να χρησιμοποιήσει ή και να εφαρμόσει το πλάνο που αναδεικνύεται μέσα από την παρούσα διατριβή, προσαρμόζοντας το πάντοτε στις δικές του δυνατότητες, ιδιαιτερότητες και ανάγκες.
Through the years, changes in human activities and lifestyle created an increase in the production of different types of waste. Health sector, could not be an exception to this development. Medical waste (MW) refers to all waste produced within healthcare facilities and are divided into two major categories, Hazardous Medical Waste (HMW) and Non-Hazardous Medical Waste (non-HMW). Rate between 75%-90% of the total MW production worldwide is estimated to be non-HMW, while the remaining 10%-25% is estimated to be HMW and more specifically, infectious, toxic and radioactive. In many hospitals, especially in developing and underdeveloped countries, the management of Infectious Medical Waste (IMW) as well as all types of HMW is not in a satisfactory level or even worse is out of control, mainly due to limited financial resources. The issue of IMW management plays an important role in the protection of public health and at the same time it is an environmental and economic challenge. Environmental, since IMW contains dangerous pathogenic microorganisms, with a complex composition which, if not properly managed and treated, could cause negative effects in the environment and public health. Economical, since correct IMW management has higher treatment cost compared to the management of non-HMW, due to the special treatment they need to undergo in order to be converted to non-HMW and became safe for final disposal to an authorized landfill. The main purpose of the present doctoral thesis was to focus on the optimization of the IMW management process in 6 wards of the Nicosia General Hospital, as well as on improving the level of knowledge, attitude and behavior of the main professional groups involved in the IMW management process. In order to accomplish the requirements of the purpose and research objectives, triangulation methodology and PDSA (Plan-Do-Study-Act) model known as the Deming cycle model were used. Data collection was carried out through on-site observations, questionnaire and relevant documents reviews. Additionally, actual infectious waste quantity produced was recorded from the studied wards, in two different periods. Based on the findings from a questionnaire conducted with 82 participants and on-site observations, it was concluded that staff knowledge regarding IMW management and the practices applied in the studied wards were moderate, indicating a need for significant improvement. The study also revealed that the additional parameters introduced and implemented as part of this doctoral thesis positively impacted staff knowledge, attitudes, and behaviors related to IMW management in the wards under study. These parameters also positively influenced the overall quantity of IMW produced, with a significant reduction of approximately 11% in the amount of IMW generated per kg/bed/day. The methodological approach applied in this study can be adopted by other hospitals in Cyprus or internationally to evaluate and potentially improve their internal procedures and practices in similar wards. Hospitals can use or adapt the plan developed through this thesis, tailoring it to their specific capabilities, peculiarities, and the needs of their wards.
